| BRS Proctor Care Case Manager |
| Member of an interdisciplinary treatment team for children (ages 6-18). Primarily responsible for the provision of case management services to children within a continuum of care and to provide appropriate opportunities for socialization and teach skills, according to client treatment objectives, in various community settings. Services are provided across multiple service settings and in collaboration with multiple service providers. Responsible for the implementation of the child and family care plan. Concrete services to client and (foster) family may include: Mentoring, behavioral and mental health interventions; modeling of appropriate parenting strategies within a Strength-based model of care; general (foster) family support and transportation services for clients as indicated by the plan of care. Provide training and guidance to members of the treatment team and transport children as needed. Responsible for the presentation of clients’ behavioral data at treatment and/or review meetings; oversee and advise adult caregivers in regards to activities of clients to help ensure they are consistent with treatment plan and/or service plan. Consult with team members, agency personnel, community professionals and (foster) family members regarding issues affecting the client. The position does include being on-call and being available to work evenings and weekends as necessary. |
